General & Family Physician 's  Response
I warmly Welcome you to HCM. I have carefully gone through your case and it seems to be due to cerebellar dysfunction. Cerebellum is the brain area concerned with the gait and balance management. Hostory you have is highly suggestive of the same.

History of Head Injury, frequent falls, concussions, diabetes, quadruple heart surgery are all suggestive of the problem in brian.

I preferably suggest you to consult a Neurosurgeon to look into the matter. While in his office, he will examine you, will run a battery of tests and will treat accordingly.

Hope it helps. If you need further assistance, contact me anytime directly from my profile.

Regards!
