What Causes Pulsatile Tinnitus?

Hi doctor, I can hear my heart beat in my ear, sometiems it goes away, but mostly it just sticks in my head when I am in quite area. When it happens, I can feel my brain move same move as my heart beat sound in my ear. Sometimes, I stay awake in bed for 1-3 hours trying to sleep because of this. Could you please tell me what you think about this? Thank you!
