What Causes Radiating Pain From Neck Down To Lower Back?

I have pains in my upper body that tend to start in my neck or shoulder and spread across my chest, ribs, sides, hips, back, and arms. I also get lightheaded and have a cracking sound when I move my neck. The symptoms come in waves and vary in length before subsiding and I have periods of no pain or dizziness. The time between attacks seems to be getting shorter and the attacks lasting longer. I have had X-Rays, EKGs, Ultra Sound, MRIs, Stress Tests, and even shoulder surgery. Everything has come back clean but the attacks continue.
