What Causes Rectus Abdominis Hematoma Along With Chronic Cough?

Hello, I was recently hospitalized for a rectus abdominus hematoma, large in size--15 cm w x 5 cm thick x 12 cm craniocaudal dimension, with active bleeding as I arrived at the hospital. I had been coughing a lot for two months previous due to sinus infection/bronchitis with asthma complications. I have previously coughed so hard that I have experienced electric shocks in my spine (when I was younger), bruised ribs (when I was pregnant), another internal bleed which caused severe abdominal subcutaneous bruising (two years ago with double pneumonia), and this most recent case. I am a top-heavy 49-year-old woman, most of my weight is upper body. 280 lbs and 5 6 , with a history of asthma, diabetes, severe spinal stenosis in lumbar spine due to spondyloarthropathy (which my Mom and my sister both are afflicted with--my Mom is paralyzed), a history of Irritable Bowel Syndrome, and increasing neuropathy with burning pain and itching and increasingly diffuse sensation in both feet in the toes and the metatarsal region, despite excellent blood sugar control over the past three years, as well as neuropathic numbness and tingling and burning on the skin over my left scapula, and increasing neuropathy in my left front thigh--possibly due to the stenosis or a blocked nerve. I also have a history of 2 years of urine tests for various reasons, most of which show a few red blood cells, regardless of reason for urine test. Kidney scans are clear with only one or two small cysts (0.57 on each kidney), with mild cortical thinning. I am wondering if any or all of the above could indicate a diagnosis of Eosinophilic granulomatosis with increasing vasculitis--hence the bleeds. I take the following medications daily: Montelukast, Flovent, Flonase, Ventolin PRN, lisinopril, pravastatin, zoloft, adderall, metformin, pioglitazone, meloxicam, as well as daily Vitamin D, Vitamin B, low-dose aspirin, and a probiotic. At the time of my hospitalization, I had completed three courses of antibiotics--amoxicillin, doxycycline, and Augmentin along with high-dose amoxicillin, in that order. I work a full-time desk job and I m not very fit, but my basal metabolic rate is and probably always has been high, and I have a great deal of skeletal muscle compared to other women of my age, height and weight. Thanks in advance for your response.

General & Family Physician 's  Response
Hi,

The hematoma is secondary to the effect of Aspirin over the clotting parameters giving enormous size hematoma.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Purva Patel,
