What Causes Recurring Fever Inspite Of Giving Tylenol And Ibuprofen In Children?

I have a two year old client who has recurring intermittent fevers up to 104 F. This has been going on for over a year. Formulas has been changed several times. duocal added to her formula made the fevers worse. along with constant retching. She has been to the ER severa ltimes for these fevers, testing is done-blood draws, chest x-rays, urine tests. all inconclusive! The fevers continue, 99 at first, slowly climbing every day until she gets up to 104! Tylenol & ibuprofen do not appear to reduce the fever. Any ideas on what to look for

Hi,

This depends on few more factors- the approximate frequency of episodes, duration of each episodes, any associated symptoms and impact of fever on her overall growth and development. If it is just once in 2-3 months, without any other associated symptoms (except for minor cold/cough), clears in 3-5 days, active and playful during episodes, feeding as usual and she is growing normally with normal weight and developmental milestone, I wont be worried.

If I were your treating doctor, apart from the above said tests, after examining for pallor, generalised lymph node enlargement, rash and enlargement of liver/spleen, I would have ordered an ESR, peripheral smear and an ultrasound abdomen (to look for any hematological / inflammatory disorder and hidden abscesses) and proceed accordingly. Your doctor will guide you. It is better to stick on a single pediatrician for further follow ups. This will help further workups needed to be systematic.
