What Causes Recurring Red Rashes On Incision Area Of Surgery?

My dad had surgery 3 months ago. A month and a half later the insicion started turning red and rashy. Surgon opened him a second time cleaned it all out did a biopsie. Test came back clean. Now 3 months into it the rash came back. It's the size is 2half dollars and draining. The surgeon is stumped. Just looking for poss answered.
