What Causes Red Itchy Spots On Penis?

I am having a problem on my penis, red itchy spots and pimple like things on it, been to the doctors and said it was scabies i took the treatment cream permethrin but it did nothing, i have been dealing with this for years now and it wont go away, it comes and goes on and off every few days, no sexual contact no masturbation, idk what it is would just like someones opinion

I really appreciate your concern, if the treatment of scabies fails to work then it could be fungal infection and this can be well managed with "Miconazole with Hydrocortisone" cream apply this three times in day, hope this information helps, take care and have a nice day.
