What Causes Scar Tissue Formation In The Vagina?

i was told by my gyno that she could feel scar tissue during an internal exam on my cervix, and that it may be the reason i have painful sex and bleed after and why when i got a pap 2 weeks ago it hurt very bad,i also have endometriosis and hpv could any of those be the reason for the scar tissue or is it from my 2 deliveries they were pretty easy though.
