What Causes Scrotum Wall Thickening?

What would be the causes of a thickening scrotum wall? My husband left scrotum has a hard mass in it. He is 48-years old, very healthy, but also has Type 1 diabetes (diagnosed two years ago). His sugar has been very high overall for the past two weeks compared to usual (200 to even 500) and some super lows (36-40), which I don't think is a coincidence. No soreness, no redness, and no fever though.

Hi,
You should get him examined by a Surgeon who would be able to tell you what the mass is. It is very difficult to say what it is without examining him. He also might require an Ultrasound to be sure. Also, you should see a Physician or an Endocrinologist to get his Sugar in control specially due to the swinging levels.
It is good that there is no redness or fever.
It could be a testicular mass or evolving Cellulitis. I suggest you get the consultation early as delay is not advised in diabetic patients.
