What Causes Severe Cramps During Menstruation?

I've been having severe menstrual cramps since I was 13, severe enough to send me to the hospital every month. I've been on birth control. I've also had two cysts and one ovarian tumor. The doctors have told me that I don't have endometriosis or thyroid disease, so what could be the cause of my severe cramps?
