What Causes Severe Headache While Suffering From High Blood Pressure?

My blood pressure usually runs 128/91 . Then today I started feeling off in my head and feels like my heart is pounding but not fast and now have massive headache and real light head, and light chect pains. So I checked my bp and it was 100/51 heart rate 71 . I had a heart attack 3 years ago. So is this a concern need to be seen by ER?
