What Causes Severe Heartburn?

I have had heartburn for the past 4 days with no relief. Prior to this I would have heartburn infrequently and the over the counter antacids would work right away. I haven t eaten anything yet today and woke up wth heartburn! I ate a lot of candy this past week ( Halloween chocolate) and I smoke a pack a day. I have stopped the chocolate and coffee hoping this would help. It s not excruciating heartburn, uncomfortable though. Should I try a different over the counter meds, apple cider vinegar w water or call my doctor on Monday? Thanks for your help.

Hello dear
I understand your concern
Heartburn could be due to acid reflux, acidity, peptic ulcer disease.
I would suggest to consult the gastroenterologist and undergo routine blood tests, urea breath test, upper GI endoscopy to rule out the cause.
Doctor will give following advises:
Tablet esmoprazole plus metoclopramide before the meal
Syrup mucaine before the meal, ginger capsule.
Do not avoid to food, take small frequent meal rather than large meal,
Take light diet like curd rice, banana, applesauce, toast, buttermilk
Avoid stress, do regular exercise like walking and yoga.
Avoid spicy/oily/fatty food
