What Causes Severe Itching On The Left Ankle?

I have a chronic itchy rash in my left ankle. Have tried many topical creams . The itch can be very intense. I have more recently been havin swelling and pain around the same ankle. Dermatologist has stated it may be lichen planus. The rash now is appearing further up my leg. Biopsies same area yrs ago with same dermatologist and lupus was ruled out. Last topical med has been betamethasone.

itchy rash are dure to
1)infective cause-
specially fungal infection.
antifungal application will give relief in this cases .

2) non infective cause
exzyma,lichen planus psoriasis etc
topical or systemic steroid will be helpful in this condition.
