What Causes Severe Lower Abdominal Pain After Trans Vaginal Hysterectomy?

I had a trans vaginal hysterectomy and left tube and ovary removed after 6 laprascopies to treat endomitriosis, I have had some pain here and there but now it has been over a year and suddenly I have had four episodes of light bleeding accompanied with moderate to severe low abd. pain. What could this be? Shoud I be concerned
