What Causes Severe Pain Below The Breasts?

I have pain which is worse when touched right below my breasts and by my ribs on both sides, as well as in my lower back. I have off and on dizzy spells and feel nauseated and I sleep a LOT, which is unusual for me. Also, when I pay $20, am I just paying it one time or am I signing up to pay $20 monthly?
