What Causes Severe Pain During Periods?

I'm 31 been on ortho tri cyclean for Months, the last seven days my period has been heavy with heavy spotting...now all day today I've been in severe pain and I've passed a golf ball sac that was clearish with a little blood with it.....don't know what to do or think...I feel like im gonna die.
