What Causes Severe Pain In Glans Penis When Aroused?

I went to a urologist a few months ago. I am 50 with typical male type issues of a relatively slow urine stream and a slightly enlarged prostate. I do not see a doctor very often and am in good overall health. I decided to see the urologist however because a more pressing issue: when my penis begins to get aroused, the glans hurts...a great deal. He thought it was prostatitis and put me on an antibiotic for 14 days which had no effect whatsoever. While at the clinic, he also took a urine sample and gave me a prostate exam. Nothing unusual was found. I did note that for a full day after the prostate exam, my testicles were quite sore on the right side. Thank you !
