What Causes Severe Pain On The Blade Shoulder To My Neck?

Hello, I have severe pain on my blade shoulder to my neck in fact it seems like having stiff and so painful to stretch...it seems connecting to the whole area of my back....it's very painful and i have gone to massage to let it gone but the pain gone just for few hours then it hurts again. May i know what's the cause of this?
