What Causes Severe Sciatic Pain From The Pelvis To The Legs?

I am 65. At 43 I had a radical hysterectomy with pelvic lymph node dissection. I was misdiagnosed for cancer but did not have it. Unfortunately I got nerve damage to the obturator nerve and to the genito femoral nerve. Too many clips (44) were placed by an inexperienced fellow at the lymph node sites. In the last couple of years I have developed sever sciatica. The pain is deep within the pelvis and travels down the leg all the way down to the foot. Right side. Sounds crazy, but often the same nerve path travels up to my mid back, right arm, neck, and side of face to head. I had epidurals on L4, L5 without much help. I had them three times. Incidentally I had a pelvic/hip x-ray, which showed cysts in the upper right corner of the x-ray. I also had CT scan, which showed three good sized cysts. I believe that I have Tarlov cysts. I read that they can press on nerves and can cause the nerve pain in the lower back and leg and hip area, and also that they can cause the pain that I am having in the upper body. I mentioned this type of cyst to a hip specialist, and to a neurologist and they both didn t seem to even know what I was talking about. I plan on seeing a neurosurgeon since whatever the cysts are, they need to come out. I hope that the doctor is familiar with cysts that press on nerves. Thanks.

General Surgeon 's  Response
Hello,

I have read through your question and would like to tell you that sciatica is a radiating pain in the legs due to nerve compression. First of all, your condition has to assessed by an MRI spine. Conservative measure s of treatment are.
1. Pregabalin or Gabapentin for radiating pain.
2. Painkillers like Aceclofenac or Naproxen.
3. Physical therapy under a good chiropractor.
4. Applying cold packs or hot fomentation.

If All these measures fail then you have to consult a neurosurgeon for surgical procedure. Avoid lifting heavy weight and bending.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein,
