What Causes Severe Sciatica Pain Radiating To The Leg?

Ongoing severe sciatic pain and periformis symptoms. I ve had injections with little help. I ve had two past surgeries, L1 through L5 rod and fusions. When I sit I m I a lot of pain radiating down the leg and up into the shoulder as well. Just want some relief.

The cause is typically some sort of nerve root compression. Typically, physical therapy helps, and if not, you should get referred to a neurosurgeon for an opinion.
