What Causes Severe Shoulder Pain ?

I am having severe pain that seems to originate under the center of my right shoulder blade. It often extends down my arm into my elbow and wrists. It varies in intensity from dull to burning and might go away for a day or so. I go to sleep with it and notice it when I wake up. Advil does not seem to help. Any suggestions?
