What Causes Severe Tiredness And Nose Bleeds Despite Being An Avid Smoker?

So I m an avid smoker, and recently I have been feeling very tired, i ve been getting sick more frequently and these sick periods have been lasting from a week to a couple weeks, my gums are bleeding, im getting a lot of nose bleeds, and I have no energy. I m also only 24 but i have been smoking since I was 13, what should I say to my doctor, and what are the potential things that could be going on?

tell your doctor how you feel exactly as you said to me, but you need to do full lab. investigations to know whats going on, as well as chest x ray
