What Causes Soft Spot On Scalp While On Hydrocortisone Solution?

My daughter is 4 years old and has a scalp condition. Tonight I was putting her hydrocortisone solution that is prescribed to her on her head. As I was rubbing it in with my fingers I felt a soft spot on her head. It is not on the top center bit rather left of center on the top of her head. It is about the size of a kidney bean. I am freaking out a little because she has not had any injuries. I use this solution a few times a week and I would have noticed it if it was there just four days ago when I did this last. I will call her doctor Monday morning but would like to know if I should take her to the er before. Thank you in advance for your help.

I really appreciate your concern, it may not be skin condition but could be something else and this need to be investigated and biopsy would be advisable or at least CT imaging can be thought, hope this helps, take care and have a nice day.
