General Surgeon 's  Response
Hi,

Considering these symptoms it may be caused by STD if there was recent intercourse. Another causes include is decreased levels of estrogen.

If Premarin is not helping then it can be related to decreased levels of Vitamin B12 or a mild nerve root compression at the level of sacral spine. Keeping the area moisturized with peanut butter will also help.

You can also start taking methylcobalamin supplements and see if it helps. Please feel free to discuss further.

I will be glad to answer the follow-up queries that you have. If there is no improvement, get swab test and it is fungus, take vaginal pessary and oral intake of Fluconazole. Keep the area dry and clean. Avoid excessive use of soap.

Hope I have answered your query. Let me know if I can assist you further.

Take care
