What Causes Spotting Despite A Normal Pap Smear Report?

Age 45, no children, regular (26-29 day) menstrual cycles, regular pap results my whole life. Now growing more irregular in recent years (early/late, some pre-period spotting) This month I have been spotting for about 3 weeks (after a 45 day cycle), with a 10 day period in the middle. I am very concerned and worried.
