What Causes Spotting Entire Time Through Active Pills And Heavy Bleeding?

Hello- so, I've been on birth control for about a year. A month ago, because of inclement weather my prescription was 2 weeks late. So, then I started again at the next Sunday after the prescription arrived. This whole 21 day active pill part of a cycle I have been spotting, and in the shower just now, a whole bunch of blood [excuse the graphic nature of this] more or less poured out of me. Is this normal, or should I see further medical care?
