What Causes Spotting Followed By Heavy Menstruation?

I m 46 years old, female. I was three weeks late for my period, took a pregnancy test which was negative. Two days later I started spotting for three days then I started bleeding heavily. It has been seven days of heavy bleeding. I have been using a pad and tampon at the same time but have to change them about every hour. I have also took notice that the color of the blood is bright red but no clots. I have no cramping or abdominal pain. I m weak, tired, dizzy when I stand up. Can you tell me what might be going on?

Uterine fibroids may cause heavier than normal or prolonged menstrual bleeding. Polyps. Small, benign growths on the lining of the uterus (uterine polyps) may cause heavy or prolonged menstrual bleeding.
