When you had unprotected sexual relation and you are not sure whether you took the post coital emergency
contraceptive or not, you have to consider the possibility of a pregnancy.
In fact, after taking post coital contraceptive pills, the following period is expected to get delayed by few days to about a week or so. Since you had unprotected sex 7 weeks ago, your last menstrual period must be more than a week before that.
Considering that the spotting you had was not a regular period, now it must be about 3-4 weeks after you have missed your period. Moreover, you are having
frequent urination and weird uneasiness in the abdomen. Though low back ache cannot be attributed to pregnancy, the other symptoms are suggesting the possibility of pregnancy. You have got following options to confirm the situation:
1. Perform pregnancy test on overnight morning first urine sample.
2. Alternatively you may opt for Beta hCG test on your blood. This is very sensitive and specific test and hence is very reliable.
3. At this stage, even
ultrasonography will directly give you a visual evidence of the pregnancy status.
Since you have
sharp pain in left iliac region associated with spotting, you have to consider a possibility of ectopic pregnancy wherein, the fertilized ovum is implanted somewhere outside the
uterine cavity, most commonly the tube. Such pregnancy cannot grow long and results into a rupture. Rupture of tube is an emergency condition requiring emergency surgical intervention with many units of
blood transfusion.
If either of your pregnancy test is positive and ultrasonography shows empty uterine cavity, please consider the possibility of ectopic pregnancy. Ectopic pregnancy before rupture can be successfully managed conservatively by medicines like
Methotrexate.
