General & Family Physician 's  Response
HI, thanks for using healthcare magic

The pain maybe related to the diabetes itself damaging the nerves and also due to the sores that are present on the legs.
His blood sugar control is good, this should help the lesions to heal but he may also need antibiotics as well

If the lesions do not heal even with a course of antibiotics, he may need a swab done of the lesions

There are medications that can be used for nerve related pain, they are commonly used in diabetics eg amitriptyline, tegretol, lyrica, gabapentin
His doctor can prescribe one of these for him to use.

I hope this helps
