Hello!

Welcome and thank you for asking on HCM!

I went through your question and understood your concern.

His symptoms may be related to epileptic seizures or to a postprandial hypotension.

Epilepsy seizures are usually non related to eating and they can occur at anytime. But it may be just a coincidence.

It seems not to be related to a cardiac arrhythmia, because the pacemaker would have changed this situation.

Have you measured his blood pressure during these episodes?
How long does he stay unconscious during these episodes?

I recommend to closely monitor his blood pressure different times during the day, before and after the meals.

A neurological consult coupled with a brain EEG to examine the brain function and rule in/out possible seizures would be necessary.

Hope to have been helpful!

Best wishes,
Dr. Aida
