What Causes Swelling And Knot On Chest That Hurts On Applying Pressure?

My husband is having upper back and chest pain on his right side. When I rube his back he has a knot and when he rolled over his chest looks swollen and you can feel a knot as well. It only hurts when he puts pressure on it or moves the wrong way. What could be causing the swelling?
