I have gone through your query and being your physician i completely understand your health concerns.
For how long u are suffering from it? Any other associated problem? like
diabetes or hypterension? Any history of TB in past or exposure to TB? What medicine u are using now? have u ever done ur ECG?
Chest tightness in a smoker patient should not be taken lightly since it may points toward heart as well as lungs.
In my opinion u should be seen by ur attending doctor to rule out any cardiac event plus
pneumothorax too
If i am ur attending physician i will get ur FBC
ESR ECG and chest XRAY . after that i will be able to offer you any treatment.
Second ur BP reading is also high. it need to be controlled. if u already taking any medicines there is need to review ur treatment.
Ankle swelling which you have mentioned need to looked at carefully since it may be due to lungs as well as heart , after then management can be initiated.
Meanwhile stay calm and use
acetaminophen to relieve pain. avoid salt intake and try to quit
smoking.
