What Causes Swelling In Hands With Muscle And Joint Pain?

I am a 33 y/o female, 5'4" and weigh 125. For the last few years I have experienced seliing in my hands (to the point I can't write), aching in all muscles and joints with hair loss and fatigue. My doctor has ran every blood test he could think of and finally diagnosed me with fibromyalgia. Went back to Dr. when I experienced a rash on my shins and face that would come and go (heat made it much worse). Dr. ran more tests thinking maybe Lupus. The only test that came back abnormal was a Anti-smooth muscle test. Referred to a Rhuemy and he said there was no evidence of that (he did no tests). I am at my wits end, please help me figure out what this could be.
