What Causes Swelling In The Testicles After Intercourse?

Hi Doc, a little over a month ago, I had unprotected sex with a 19 year old girl. I am 21 years old. It lasted for 30 seconds-1 minute as I realized that I had no condom on. That was my first time having vaginal intercourse, little sexual encounters throughout my life apart from a couple of blowjobs in the past as I am quite conservative and practicing my Catholic religion.The first 7 days after the event, I felt a slight discomfort in my testicles, not painful though but it did feel slightly different. Not enough to lose sleep over however. I also believed it swelled slightly but everything went away within a week. 8 Days after exposure, I went to get tested through a urine test and blood test for chlamydia and gonnorhea and for any STD s that can be caught in my bloodstream. I told the nurse at the clinic about my testicles but she believes that they can also be anxiety induced (as I am slightly anxious when it comes to these matters). I was told 10 days later that I tested negative and so I don t have chlamydia or gonnorhea. However, Herpes, Syphillis and HIV require to be tested 6 weeks to 3 months after exposure. Last night, there was a pimple on the lower penile shaft. It looked a small lump, slightly red but looked like a pimple that I usually see around my inner thighs away from my genital area. It popped like a pimple with the white zit like pus but with a bit of blood. The lump decreased to around the size of a very small pea, and is still red with hard smooth surface (does NOT appear to be a wart or blister like sore). The pimple does not bother me but the fact that it is on my penile shaft does as this is the first time I see there. Can this pimple have any relation to what happened when I had unprotected sex? Could this be a chancre sore, a symptom of syphillis or any other STI or is it simply a pimple, or cyst or ingrown hair?

Hi,

This type of pain and location is suggestive of epididymitis which is inflammation of part of the testicle and this not uncommon in males especially in young age.

This is usually treated with 10 days of antibiotics, usually ciprofloxacin and there are no permanent consequences.

Masturbation and intercourse should be avoided till pain persist, if possible since I may prolong inflammation course.

Some more serious conditions such as tumors or torsion are rare but in doubtful cases you should do Doppler ultrasound or scintigraphy.

In this particular case, I don’t think this is required and this should improve on antibiotic therapy.
