What Causes Swelling , Pain And Tenderness On The Upper Surface Of Foot?

My MRI of Lumber spine shows:L3/4 and to a lesser extent L2/3 posterior disc bulges are seen effacing the anterior epidural fat,indenting the ventral aspect of the thecal sac and encroaching upon the neural exit pathways bilaterally at the former level.L4/5 posterior and left posterolateral disc bulge are seen effacig the the anterior epidural fat ,indenting the ventral aspect of the thecal sac and enchroaching upon the left neural exit pathway.
MRI Right foot:Right 2nd middle phalanx altered marrow signal intensity likely representing Bone Marrow Contusion with overlying subcutaneous edema,however, inflammatory process could not be totally excluded for clinical correlation.
CT Angiographic examination of the peripheral arterial system:
Relative attenuation of the anterior tibial artery distally
Small bony projection from the right lower fibular medial surface is seen pseudoarticulating with tibia with relative widening of the distal tibio fibular joint.(Bone exostoses) after Pott,s fracture since 40 years ago.
Complaint: pain, tenderness, swelling of the upper surface of the second toe of right foot ,with numbness and tingling of planter surface and sense of pebble on walking.recently a numbness of the left foot in the three lateral toes occurred and pain in the lower back after long setting or standing.
There are a history of trauma and stress fracture of second metatarsal bone in the second right toe .
uric acid was always high ,and AST enzyme of liver reach 62 ,I am hypertensive under concore 5 daily & zyloric 100 daily and colchicine 100 mg daily . my age is 67 years old , work :General Medicine
