What Causes Swollen And Enlarged Lymph Gland?

I have extreme pain in the right side of my waist/hip bone and a left swollen lymph gland that has gone down some but is still enlarged to the point of being visual. Saw Dr who did not know but said to watch for fever and the node getting larger but that was it. I have no other symptoms.
