Swelling of feet and lower legs can occur due to heart ailment like
heart failure.
However, normal cardiac investigations rule out the possibility of heart ailment.
I am of the opinion that there is some local cause of swelling and development of red spots.
I suggest you to get a complete hemogram along with platelet counts.
Low platelets can lead to red-purple spots on body.
The swelling could be due to some disorder of lymphatics - inflammation or damage to the lymphatics.
Swelling caused by congestion of lymphatics usually relieves after raising the legs.
If the swelling is not relieving after raising the legs, then possibility of lymphatic abnormality needs to be ruled out.
Hypothyroidism also leads to swelling and thickening of skin.
Thus, clinical assessment is necessary to look for any of these disorders and relevant investigations are required for definitive diagnosis and thus management.
