After having gone through the history and the prescribed medications, tablet Actos seems to be responsible for the swollen ankles and feet.
Actos (Pioglitazone) is commonly prescribed as an add on therapy for the treatment of uncontrolled type 2 diabetes mellitus.
Peripheral edema (collection of fluids in the subcutaneous tissue) is one of the important side effects of Actos causing swollen ankles and the feet. If the treatment with Actos continues, it may cause heart failure.
I advise you to consult your treating physician for the dose reduction or to discontinuance of Actos must be considered.
Hope this will be helpful.
