What Causes Swollen Feet/hands And Constant Heartburn?

My boyfriend experienced pain in his arms for some time, then his knee was in pain due to an old injury, He constantly has heartburn and is always thirsty, now he his feet are swollen and his hands have pins and needles, please help as I cannot get him to a doctor.
