What Causes Swollen Knuckle?

I hit a knuckle (on my left hand) against the side of the house with lot of force as I was trying to kill a snake with a hoe. I iced the hand and keep it elevated. After a week it is still swollen. When I curl my fingers aound for a fist, there is a jump as the finger comes back in place. Is the finger dislocated perhaps? Should I see a doctor?
