Hi,
The brief clinical history that you provided is suggestive of an infection in the throat. White patches on the back of the throat suggest the presence of some pus due to bacterial infection. The ear pain is likely to be a referred pain due to the throat infection (because they share the same nerve).
Mono is unlikely as you have had it before however the possibility of strept cannot be ruled out.
You may need a course of an antibiotic. Please consult your doctor and follow his/her advice.

In the meantime gargle with warm salt water thrice daily. Anti-inflammatory medications could also be helpful.

Hope this helps.

Regards,
Dr. Prasad
