What Causes Testicular Swelling With Hypersensitivity?

my male cousin age 24 is having swelling of his testicles along with hypersensitivity, leg pain which I believe is Sciatic\Piriformis pain, also light headed and dizzy he has had a lot of test and they found a herniated disc but told him it could not cause the swelling. pain is excruciating so far they have not sent him to a neurologist should he go to one and can the herniated disc be the problem and or any kind of nerve root irritation also how about his pudendal nerve being affected i guess he needs to know who to go see
