Cancer would be the first thing he needs to be checked for with his history. Tracheal or laryngeal cancer could cause such symptoms, and the combination of chain smoking and alcohol greatly increases the risk. He needs to be seen promptly be an ear nose and throat specialist and have this throat and vocal cords examined. It could be just some kind of infectious process that will clear up, but given his history, it is critical that a cancerous process be ruled out immediately. These cancers can be treated effectively. Hope this helps.
