What Causes Thumbnail Discoloration With Shooting Pain?

For about 12 years I have had an area under my thumbnail that looks like it was injured by a hammer - the nail appears to be blue/black but is not. That area is ultra sensitive to cold and if I bump it. The pain will shoot from that area on the outside right of my thumbnail, up my arm. What could be causing this?
