What Causes Twitching In Lower Back And Face After A Mild Concussion?

CONVERSION DISORDER DISMAY--- Hi my name is Karen Smith. I live in Doylestown, Pa. I was in a minor auto accident (not my fault) on 7/2/15 which caused a minor concussion. I was first seen by my PCP who diagnosed me with a minor concussion (7/7). When my symptoms did not go away I asked if I could work 1/2 days starting 7/14. When my symptoms (bad headache, neck ache, dilated pupils) did not go away, I was told to stay home by my work until my Dr. signed a release. After going back to my PCP on 7/16, I was told to go see a Neurologist & get an MRI. My Neurologist then ordered me to see a Vestibular Therapist after seeing nothing on my MRI w/o contrast . After I started VT, I noticed slight twitching in my eyes, mouth, forehead, & shoulders. On the evening of 7/31 it got so bad that I needed help walking. My Husband said it looked like I had Parkinson s disease. After that night, my twitching had moved to my lower back, legs, face, & upper chest, leaving me unable to walk correctly, drive, or work. I also noticed I had hypersensitivity. Upon my appointment with my Neurologist & after seeing my movement trouble, he ordered me to seek help from a Psycho Therapist, stating my troubles were all in my head . After showing my condition to him my VT asked me to meet with his colleague on 8/11, who was so troubled by my movements, she contacted my Neurologist who in turn ordered me to go to my local ER. I had blood tests done & then released after being told again to go see a PT about my condition. It was only after seeing my PCP on 8/14 that I was told the Drs. thought it was either Astasia Abasia or Conversion disorder. On 8/18 I went to a Movement Disorder Dr. who again told me to see a PT & that it was all in my head. On 8/26 I met with a PT who I am still seeing to this day. My PT ordered a full blood panel and a MRI with contrast (11/19) which showed nothing. My PT has said my movement troubles are due to some issues from my past & also because I never put myself first. I have seen no improvements in my condition & upon seeking legal help, I have kept written and video logs. We have found that I get worse after any prolonged physical activity (video showing me unable to talk), & still cannot drive or work. We have tried to be seen by Teaching Hospitals in Philadelphia, Pa but have been turned away because of my open claim or because it wasn t sports related. We have done research on our own & found that my hypersensitivity symptoms mirror Spasticity & have read up on the lingering effects of concussions. I am desperately looking for someone who will help find out what is really going on so I can get back to my life. I mapped out all of my medications below: Methocarbamol 750 mg - caused nausea. Stopped after one dose Clonazepam .5 mg - stopped after one week Quetiapinc (seroquel) - still taking today. Started at 25mg 2X a day, now 300mg every evening Ropinirole .25mg - still taking 2 every night Larazepam .05mg - still taking 1/2 tablet 3X a day Pristiq 50mg - still taking 1 every day

General Surgeon 's  Response
Hello,

* As per my surgical experience, the movement disorders of grave level disturbances do indicate most possible damage to motor control area as motor cortex, premotor cortex or motor association area of the brain regions.

* However, it is better to co relate the things with clinical examination
including

- sensory
- motor functioning of whole body
- clinical reflexes
- plantar response
- cranial nerve examinations with the report findings of MRI and others.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Bhagyesh V. Patel
