What Causes Vaginal Bleeding During And After Intercourse?

i recently became sexually active with my boyfriend. We've had sex about 5 times now, and the first time I bled which was understandable...but every time after that I've bled as well. This most recent time there was quite a bit of blood...and it kept bleeding after a few hours. What could be wrong, and how can I try to help it?
