What Causes Vaginal Bleeding During Pregnancy?

HI Dr my wife has gone with ET and every thing was fine and the next review was done in the 6th week the fetus has heart beat of 182 and growing well it was found by TVS. Same day in the eveningyear wife found a red spot in her undergarment another spot with 3 hours then we report to the IVF center than they said to start Sylate 500 Mg every 6 hourly we gave only three dose and next day evening a massive bleeding happened and when we went for USG they said the fetus is there but no cardiac motion is detected. Hence kindly suggest what happens wrong and was it due to TVS examination or due to sylate 500.
