What Causes Vaginal Soreness?

Im on the nexpolion birth control and I get my period every 3-4 months and they last about a month but this month it s the ending of my period my im having fleshy little pieces come out with brown blood but also my beginning of my vaginal hole is very sore and hurts to wipe and have sex especially thank you if you can help
