What Causes Vaginal Spotting And Severe Abdominal Pain?

Hello Doctor, I experience patchy blood discharge in my pant following a sharp pain in my abdomen. This usually occurs 10 days starting from the first day of my period. My flow is usually 3 to 4 days. In between the time I menstruate and when I observe the patchy discharge, the smell that comes out during intercourse with my partner is usually very very foul. The discharge from the intercourse is usually light brown and has the smell of rotten blood. what could this be? I have also done BTL.
