What Causes White Clot Vaginal Discharge On First Day Of Period?

Hello, I had a white clot about the size of a quarter during the first day of my period. I am on birth control pill and also I experienced bad cramps that hurt so much I could hardly breath. Could you please tell me if this is normal or if there is something that I should do about this. thank you.
